The generic indemnity form is suitable for any facility wishing to exempt itself from liability. The details of the participant is left blank for the indemnified party to later fill in.
The indemnity form is suitable for 2 instances: the first is where the participant is a minor, in that case the parent/guardian will supply his/her details and consent to the participant visiting the entity wishing to be indemnified and accept the terms for example that the entity is hereby exempt from liability due to injury etc to the participant's child; the second is where the participant is an adult and is therefore requested to accept the terms for example that the entity is hereby exempt from liability should the participant sustain some or any form of injury.
The living will is made by the testator (who is a person making a will) stating that should he/she be incapable of making decisions, then he/she requests that he/she be allowed to die or given medication to alleviate any pain even if it hastens death. The living will exempts doctors and family members from liability when carrying out these orders.
